We have spoken about this subject matter before on the list.
I have a couple of links to send out on this subject that I pulled
from here...

Mobile (QR) Codeatron from Pukupi

Quel - QR Code Generator (in Japanese)

They are both basically forms that you enter in the various details,  
and then
it spits out QR Code in a variety of carrier formats... I am not aware
of one that would do this for an source Image as the input file, but  
you could
easily include the images URL as the data for your QR Code, in this  
case, the
device scanning the QR Code would load the image, if it was  
online/web/wap...
