I often wondered about this issue.
Is it unwillingness at the manufacturers?
Is it just because they use the phones so much that they don't see how
difficult they are?

In general you can say that the userinterfaces on phones are not very well
thought off.
I've used Siemens, Nokia, SonyEricsson, NEC, Mitshubishi and Sharp phones
and they all have a hard to understand logic behind their userinterfaces.
For instance, new Nokia's with Symbian are so difficult to understand that
it takes lot's of time to even send a single SMS, not to speak of MMS.

On a Sharp it's even difficult to make a simple phonecall!!!
You select someone from the addressbook and then you have to press the
button again to actually make the call. It still confuses me now and then.
